We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.+ **Facility:** BayCare HomeCare+ **Location:** Bradenton, FL+ **Status:** **Full Time**+ **Shift Hours:** **8:00 AM - 7:00 PM / Friday - Monday**+ **Weekend Work:** Every+ **On Call:** Yes+ **Home visits in assigned service area:** **Manatee County****BayCare offers a competitive total reward package including:**+ Benefits (Health, Dental, Vision)+ Paid time off+ Tuition reimbursement+ Mileage reimbursement+ 401k match and additional yearly contribution+ Yearly performance appraisals and team award bonus+ Community discounts and moreThe **Home Health RN** provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. **Responsibilities include:**+ Oversee the activities of the multi-disciplinary team that provides care to the patient+ Monitor the provision of therapeutic services through written and verbal communication with all disciplines+ Ensure that all visits scheduled and provided are authorized and covered by physician orders+ Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits+ Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor**Required Certifications and Licensures:**+ BLS (Basic Life Support)+ RN (Registered Nurse) License+ Driver's License State of Florida (Florida driver's license is required within 30 days of hire)**Education:**+ Required Associate's or Diploma Nursing+ Preferred Bachelor's Nursing**Experience:**+ Required 1 year Registered Nurse+ **Preferred 1 year Home Care**Why join BayCare HomeCare?
