A day in the life of this role:
The Primary Care Access Clinician at Outer Cape Health Services ("OCHS") ensures patients of all ages have access to comprehensive same-day care, chronic disease management, and temporary coverage during primary care clinicians' leaves of absence (planned and unplanned). They provide routine and urgent medical care as needed. While not assigned to a specific patient panel, they assist with panel management needs of primary care patients as directed by the medical director. The medical director will also coordinate duties and collaborate with the population health medical director/Chief Population Health Officer to assign site-based population health care visits when coverage and same-day access needs are not in demand, focusing on health promotion and preventive medicine in line with the Patient Centered Medical Home model of care.
Summary of duties:Serve as part of a Patient Centered Medical Home team to ensure consistent coverage and access for OCHS patients to primary care and same day care medical services including regular communication with the primary care clinicians and clinical teams regarding the health of the patients managed.Supervise other health care professionals such as staff registered nurses, medical assistants, phlebotomists and other allied health care staff, as appropriate, to ensure the proper delivery of medical care to patients.Collaborate with the practice manager/director of operations and the medical director regarding daily/weekly schedule which will change depending on coverage needs.Medically manage or support the care of emergency, urgent, and other walk-in patients and cooperate with site staff in managing such needs.Keep accurate, complete, legible and timely medical and other records and files as required by OCHS' policy.Participate in peer review and patient care audits for quality assurance and utilization review to ensure an excellent standard of care.Participate in On Call duties as assigned.Maintain knowledge of current developments in specialty to enhance professional expertise including maintenance of Board Certification and completion of continuing medical education credits as needed to maintain licensure.Maintain the highest standard of professionalism toward colleagues, patients and all OCHS stakeholders.Participate in clinician and staff meetings as set by OCHS' leadership and management teamsPerform other related duties as reasonably required or assigned.Adhere to OCHS' policies and procedures.What we need from you:
Family Medicine Clinician with Advanced Practice / Master's Degree in Nursing or Physician Assistant training.Valid Nurse Practitioner or Physician Assistant license to practice medicine in the Commonwealth of MassachusettsBasic Life Support (BLS) certificationExperience working in fast paced ambulatory care settings. FQHC/Community Health training/experience preferred.Possess a valid federal Drug Enforcement Administration number and any other required state public health certifications with respect to prescribing privileges.Certified clinician and accepts assignment under the Medicare and Medicaid programs, and a qualified clinician with respect to such other third party payers with whom the Health Center may do business.Is not excluded by state or federal authorities from participating in any health care programs, including Medicare and Medicaid. Is not penalized with civil monetary fines or assessments, or administrative or criminal sanctions for a material violation of any federal or state law involving Medicare, Medicaid, or any other federal or private third-party reimbursement program.Maintains full privileges as a member in good standing of the Health Center's Medical/professional staff and comply with all by-laws and rules and regulations thereof.
