Job Description
PURPOSE OF POSITION
The Psychiatric Nurse Practitioner will be a member of the interdisciplinary team, whose purpose is to evaluate, treat, and plan for the aftercare of patients.
AREAS OF RESPONSIBILITIES
Adult OPD
SUMMARY OF DUTIES AND RESPONSIBILITIES
Direct Patient CareProvide direct care to patient assigned to the CATCH team, including appropriate and timely care and documentation.Complete admission assessment or consultation, with a focus on comprehensive risk assessment for all new intakes, and re-assessment of patients in a timely fashion consistent with policy.Conduct appropriate outreach and relevant documentation as per policy.Complete treatment plan documentation consistent with policy and appropriate to patients risk level.Prescribes medications indicated for the treatment of Medication Assisted Therapies for Substance Use Disorders including buprenorphine and naltrexone.Participate and conduct inter-disciplinary team meetings as per the standard of work and policy.Ensure appropriate use of de-escalation, and participate and provide leadership in outpatient behavioral codesParticipate in medical codes in the clinic until the Code Team arrivesWork with all teams to help patient move through appropriate level of careAppropriate risk stratification and re-assessment of risk levelConsideration of referrals to community resources like AOT, ACT etc.Participate in discharge planning and including referrals to alternative levels of care.Collaborate and coordinate with primary care and other medical specialistsParticipates and conduct tier reviews as per policyUtilization ManagementParticipate in utilization review and management.Maintain focus on key strategic priorities including patient experience, access to care, increased market share, and improving financial stability
Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and2. Current certificate as a Nurse Practitioner issued by the NYSED; and3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
