Highlights
Community Physician Network is seeking a Family Medicine physician to join our outpatient practice in Elwood, IN.
FM Practice plus Inpatient
Shared call schedule
Patient Centered Medical Home practice model
Electronic Medical Record (Epic)
Ability to practice in a smaller community while having the support of a large network
Physician-led organization that promotes and supports physician leadership
Multi-disciplinary physician group of 1300+ providers
One of the largest primary care networks in the state
Fully-integrated EMR system (EPIC)
Licensure & Certifications
Board certified/board eligible in Family Medicine.
Licensed in Indiana or eligible for medical licensure in Indiana
