We are looking for ambitious and energetic leaders to join our team in the Channelview as a full-time,

Nurse Practitioner . The Nurse Practitioner functions as a clinical provider maintaining and promoting the health of the patient requiring acute care. Affects direct patient care according to the nurse practice act of the State of Texas.

Responsibilities:Provide medical services to patients, inclusive of all functions necessary to provide medical care under the direction of the physicianConducts comprehensive health assessments (initial, ongoing, discharge), identifies normal/abnormal health characteristics, develops plan of care, initiates appropriate interventions, and evaluates care outcomes on defined patient populations(s)Performs delegated functions under physician supervision, such as differential diagnosis, use of prescriptive authority, and treatment according to practice guidelines, protocols, and/or standing orders. Accepts accountability for outcomes in health careUses critical thinking skills and clinical judgement to work autonomously as defined by the Nurse Practice Act. Collaborates with the Physician and healthcare team members to manage/coordinate patient careRecord accurate consultation data in patients' recordsCharting in the patient's medical record is routinely to be completed on the same day as the patient's visitInstigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reportsProvide safe, evidence-based, cost-effective, individualized patient careContribute to the practice achieving its quality targets to sustain the high standards of patient care and service deliveryWork collaboratively with colleagues within the practiceFollow all internal policies and procedures; be accountable for completion of all operational tasks according to departmental guidelines to ensure appropriate clinical processing/documentationSee patients in a timely and efficient manner, stay reasonably on time for scheduled appointments in practice throughout the day, unless exceptional circumstances occurMaintains current licensure and credentials. Remains current in practiceResponds and comply to requests for information including sending faxes and e-mailsReport statistics as requiredProtects patient and family privacy rights and maintains confidentiality of patient records in accordance to policy and procedure and HIPAA requirementsFollows all safety rules on the job. Reports all accidents promptly and corrects minor safety hazards. Communicates with peers and management regarding any hazards identified in the work placeTravels as necessary to other facilitiesPerforms other job-related duties and special projects as assignedRequirements:
