Nurse Practitioner or Physician Assistant – Post Acute Care (FP, IM)
Opportunity for a Family Medicine or Internal Medicine Nurse Practitioner or Physician Assistant to join a post-acute care facility.
Full-Time
Schedule:
Monday - Friday
Key Responsibilities:
Perform primary care in a post-acute setting for short term and long-term patients
Manage chronic disease, diagnosis illness and medical conditions, and implement plan of care
Order and interpret lab results
Prescribe and monitor medications
Consult with physicians and coordinate patient care with other healthcare practitioners
Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
Build relationships with your patients, patients’ family members, and facility staff to facilitate effective treatment delivery
Promote healthy lifestyle changes
Requirements:
Minimum of 1 year experience in primary care
Post-acute care experienced a plus, but not required
Active and unrestricted Washington NP or PA license required
Active and unrestricted DEA required
Compensation and Benefits:
The compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential
Compensation range $115,000 - $167,000
