Join UT Southwestern as an Interventional Pulmonary Coordinator (RN) to expand the growth of our IP program and to help with our commitment to deliver compassionate care and all-around excellence. The RN Program Coordinator provides coordination of care and disease management longitudinally to patients with chronic condition(s) or episodic care for patients requiring advanced bronchoscopy or pleural procedures to achieve optimal outcomes. This position will be responsible for intake of urgent referrals, requesting outside records, assisting with scheduling of clinic appointments/testing/procedures, coordinating patient care across multiple areas including the outpatient clinic, inpatient/outpatient procedure room, and the community to provide continuity of patient care across settings. The ideal coordinator is someone who has experience in Pulmonary, demonstrates a strong work ethic with a positive attitude, thrives in a fast-paced team environment and is self-driven.

Experience and Education

RN, BSN required; Three (3) years Nursing experience. Pulmonary experience preferred. Basic Life Support (BLS) certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) required.

Job Duties

Serves as contact and facilitator to receive urgent referrals and patient information.Collaborates with physicians and other health care providers to coordinate the care for patients from referral/consultation to discharge/follow up.Coordinates and ensures all patient scheduling including consultation, diagnostic and treatment procedures.Gathers information for history and physical.Performs and documents evaluations and assessments of patients.Coordinates and assists in ordering labs, and diagnostic procedures.Communicates with third-party payers regarding issues of medical necessity and benefits coverage for their patients in the interventional pulmonary program.Educates patients and families regarding all aspects of disease processes and preparation for procedures.Coordinates preparations for any necessary surgical or interventional procedures.Assists with coordination of in-hospital and post-discharge planning and education for interventional pulmonary patients.Assists in managing transitions of care across care setting, ensuring optimal communication and planning. Identifies barriers to receiving care and facilitates solutions.Collaborates with interventional pulmonary faculty, pulmonary faculty, advance practice providers, other coordinators (oncology, cardiology and cardiothoracic surgery, transplant, etc.), nursing staff, and other health disciplines to facilitate meeting patient and family needs.Provides follow-up phone calls to support patients and their families with any questions they may have.Updates clinical knowledge and skills through consultation, formal and informal education, and review of literature for self and clinical team.Provides leadership for process improvement related to the care of these patients.

Knowledge, Skills & Abilities

Computer skills including EMR (EPIC); Basic Excel.Presentation skills, comfortable presenting and speaking to data and processes in multidisciplinary meetings.Organized and ability to multitask.Constant walking, standing, bending, turning, and stopping. Pushing and pulling heavy objects such as patient beds. Must be able to help lift and turn patients.

Working Conditions

Work is primarily performed in an outpatient setting. However, there will be hospital components such as rounding on patients with the team.Any qualifications to be considered as equivalents in lieu of stated minimums require prior approval of the Vice President for Human Resources Administration, or his/her designee.
