Position InformationRutgers University Behavioral Health Care (UBHC), established in 1971, offers a full continuum of evidence-based behavioral health and addiction services for children, adolescents, adults, and seniors throughout New Jersey. UBHC's 1,060 experienced behavioral health professionals and support staff are dedicated to treatment, prevention, and education.Rutgers The State University of New Jersey is seeking a Habilitation Counselor in the Department of Outpatient Treatment - Certified Community Behavioral Health Clinic (CCBHC) Case Management of Rutgers University Behavioral Healthcare.The Habilitation Counselor provides community-based case management services for individuals served to include assessment, treatment/recovery planning, linkage and coordination of care. This position will focus on individuals within the Just In Time scheduling process within the CCBHC programs.Among the key duties of this position are the following:Supports individuals to gain access to needed medical, social, education, mental health, addiction, co-occurring and other services.Links individuals served to needed services including: benefits, medical, dental, mental health, co-occurring services, self-help, employment, legal and housing.Assists with scheduling appointments with the individual served and when necessary accompanies the individual served to appointments.Demonstrates the ability to engage and establish a working relationship with, and provide services to individuals who are diagnosed with a serious and persistent mental illness and co-occurring disorders who present engagement challenges.Utilizes Motivational Interviewing skills to establish a working relationship with individuals served.Provides services that are Person Centered that assure that the person's expressed needs and strengths drives the service Plan and service delivery.Collaborates effectively with members of the department, and with external departments, staff, referral sources, other service providers and social support networks to provide services and rehabilitative care for consumers and families.Maintains appropriate clinical records according to the time frames and quality guidelines established by UBHC policy and applicable accrediting organization standards.Understands and adheres to Rutgers' compliance standards as they appear in RBHS's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.Keeps abreast of all pertinent federal, state and Rutgers' regulations, laws and policies as they presently exist and as they change or are modified.QualificationsMinimum Education and Experience:Bachelor's Degree in the mental health field or other appropriate human services related field plus one (1) year of relevant mental health experience.Certifications/Licenses:Applicants possessing a degree in Social Work must have an active New Jersey certification/licensure as required by the State Board of Social Work.Valid driver's license.Preferred Qualifications:Previous community-based outreach experience is preferred.Bilingual skills (English/Spanish) preferred.Physical Demands and Work Environment:Ability to speak clearly and write proficiently. Hearing is essential.The ability to focus on the task at hand despite numerous interruptions.Must be able to sit for extended periods of time and document into an electronic medical record.The employee spends much time driving, walking, standing, bending and talking with consumers in the community located throughout the County.Transport of consumers in state vehicles.The employee can expect to lift up to 30 pounds.The work environment includes both office-based services and community outreach. Individual wellness checks and crisis intervention required. Subject to unpredictable situations, patient crises, to include behavioral, medical and psychiatric. Occasionally exposed to patients exhibiting assaultive behaviors. Exposure to blood borne pathogens that requires use of personal protective equipment.Special Conditions:May work at least one (1) evening per week and one (1) weekend day per week.Must possess own vehicle to outreach individuals served and attend internal and external meetings.
