The
Inpatient Hospice Nurse
provides nursing care to patients who require GIP level of care for end-of-life symptom management and pain control in collaboration with the Stanford Hospital inpatient team. This RN will be on-site at Stanford Hospital and requires a high degree of courtesy, tact, and knowledge of services. This is a per-diem position, every Sat, Sun. & Mon.

Essential Duties & Responsibilities include
(but are not limited to):
Perform initial and ongoing hospice eligibility assessment and consultation with Hospice MD in collaboration with Stanford Hospital and in accordance with CMS guidelines
Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions.
Perform evaluation and coordination of social work and spiritual support services needed by patient/family.
Coordinate the implementation of the plan of care and ensure care coordination with the inpatient team.
Education/Experience :
One year experience as a professional nurse in the past three years or; Bachelor’s degree in Nursing from an accredited School of Nursing.
Experience in acute care/ home health and/or hospice setting preferred .
Certificates, Licenses, Registrations:
Current license as a registered professional nurse in the State of California.
Public Health Nursing certification highly desirable.
HPNA certification preferred.
Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
Pay Range:$58.00 - $72.00 hourly
