Responsibilities
Provides preventive, urgent care, and primary care health services
Performs clinical assessment of patients, including ordering appropriate diagnostic evaluations and plans a program of care.Provides verbal and written education and counseling to patients, families, and caregivers.Plans follow-up care, including requests for outside consultations and evaluations.Maintains patient records necessary for clinic reporting, billing, and quality improvement.Functions potentially in a variety of settings, including health centers, skilled nursing facilities, supportive housing sites, and in clients' homesParticipates as a site clinical services team member and attends clinical team meetings.Provides specialty care services, clinical case management, and team participation in designated programs (e.g., HIV services, women's health, house calls, diabetes, Bridge, etc.)Coordinates care with other health team members, including case management and behavioral health staff.Acts as an advocate for the patient and liaison with outside agencies as appropriateParticipates as an active member of the LifeLong Clinicians Association and abides by its Code of Conduct.Assists in supervising and teaching medical residents, students, or clinical fellows.Actively participates in internal quality improvement teams and works with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards.
