Orientation: 40 hours @ $0 non-billableTrave Pay Package Stipend Weekly (Non-Taxable) 1617 Weekly (Taxable) 1400 Total Weekly Gross 3017 Position Overview: We are seeking a Registered Nurse Case Manager with recent Acute Care Experience who is proficient in using InterQual guidelines and has a strong background in Care Coordination and Discharge Planning. The ideal candidate should be able to work independently, handle complex cases involving diverse patient needs, and participate in the development of treatment plans. Roles & Responsibilities: Apply Admission Criteria (InterQual) and ensure competency in InterQual guidelines. Assess patients and coordinate care, including discharge planning. Manage cases involving adoption/foster care, substance abuse treatment, homelessness, mental health, and protective services. Organize and participate in family meetings. Develop and implement treatment plans in collaboration with the healthcare team. Handle the logistics of arranging DME, SNF, and ambulance transport independently. Work with HMOs, IPAs, and other insurance entities.
