Job Description - RN PRN Nurse Case Manager I/II - Emergency Department (230000EX)DescriptionThe Case Manager I/II Emergency Department has an understanding of emergency care, disease processes, utilization management and payer requirements. This position is responsible for taking steps to 1) ensure appropriate ED utilization; 2) place patients in the appropriate status; and 3) meet patient's care coordination needs and mitigate barriers to care. All nursing practice is based on the legal scope of practice, national and specialty standards, Children's National Policies and Procedures, and in accordance with all applicable laws and regulations. The Professional Model of Care requires registered professional nurses to be responsible and accountable for their own practice. Children's National supports the development of RN relationships within the community; specifically those relationships associated with the health and well being of the community at large.The Ambulatory Nurse Case Manager for the Emergency Medicine and Trauma Center comprehensively and actively maintains the care management and coordination needs of payer defined and/or payer enrolled populations of patients who meet designated inclusion criteria. This includes activities such as assessments, formal Case Management care plan development, establishing goals and intervention and monitoring/tracking. Through these activities, the Ambulatory Nurse Case Manager will partner with physician practices and the care management team related to the clinical and care coordination needs of the patient, as well as work with payers and community resources to develop and facilitate effective, efficient care delivery options for the patient across the continuum of care.The Emergency Medicine and Trauma Center at Children’s National is the only Level I Pediatric Trauma Center in the Washington, D.C., metropolitan area, as verified by the American College of Surgeons. As the only freestanding children’s hospital in the area, Children’s National serves as the regional referral center for pediatric emergencies.As a Level I Pediatric Trauma Center, we are uniquely qualified to provide emergency care for children, from the design of the department to our highly skilled professionals who understand the special needs of younger, smaller patients.QualificationsMinimum EducationBachelor's Degree Plus Associate's Degree in Nursing if no BSN (Required)BSN (Preferred)
