Position SummaryThe outpatient dietitian will work with outpatients to improve their health and well-being. The outpatient dietitian is responsible for providing quality health education to individuals at assigned site(s), the community, and staff. They will provide Medical Nutrition Therapy (MNT) counseling to individuals and groups, creating nutrition care plans using the Nutrition Care Process for patients with various medical diagnoses such as diabetes, chronic renal insufficiency, metabolic syndrome, and heart disease. Responsibilities include individual and group counseling, medical nutrition therapy, chronic condition self-management, and wellness education. Other responsibilities may include nutrition research, presentations, wellness classes, and authoring nutrition department policies and procedures.

Essential Duties and Responsibilities

Assesses patients’ nutritional risk/health status according to Academy of Nutrition and Dietetics protocols.

Partners with patients, families, and caregivers to develop and implement a plan of care for nutrition/health education and counseling.

Provides individual and group medical nutrition therapy, counseling, and support for prevention and management of chronic health conditions.

Works collaboratively with care team members to identify patient needs and make recommendations.

Documents care plans in the medical record within 48 hours of patient encounters.

Develops and approves nutrition protocols for patients.

Assists in the development and maintenance of instructional materials on nutrition and dietary guidance.

Leads and facilitates classes for the organization’s Diabetes Self-Management Education (DSME) program, Diabetes Prevention Program (DPP), and Sweet Success Gestational Diabetes Management Program.

Participates in professional development activities and follows Hospital and Department policies and procedures.

Provides excellent customer service and shows compassion to all patients, visitors, and co-workers.

Keeps manager and team informed of progress, problems, and plans.

Accepts and performs other duties as assigned.

QualificationsRequired

Current certification as a Registered Dietitian in California.

BLS – Basic Life Support Certification – issued by AHA-American Heart Association.

Skill in establishing and maintaining effective working relationships.

Knowledge of evidence-based medical nutrition therapy and dietary principles.

Ability to track information and prepare reports.

Strong communication skills.

Ability to take initiative and work independently.

Strong time management and prioritization skills.

Preferred

High School Diploma or GED.

Physical RequirementsWhile performing the duties of this job, staff is regularly required to sit, stand, walk, talk, and/or listen. Good vision is needed to read schedules and enter accurate data. Staff must have good general health and demonstrate emotional stability.

Able to lift up to 30 pounds.

Stand, walk, and move 50% of the day.

Use proper body mechanics when handling equipment.

Compliance RequirementsComplies with Chinese Hospital Compliance Handbook including Code of Ethics and all applicable regulations.

Base Pay ScaleStarting at $38.28 – $49.95 per hour. The salary of the finalist selected for this role will be set based on a variety of factors.
