ICC Services: Families requiring services please contact Perform Care at 1-877-652-7624. The coordinator on the telephone will direct you to the appropriate level of care and let you know if you or a member in your family meet the criteria for services through DCSOC. If you do not meet the criteria for IIC, you will be referred first for a bio-psycho-social assessment. At that time you may request a Therapist at Divine Professional Services.
