Our mission is to provide equitable access to innovative care that improves health, recovery and growth at all stages of life.POSITION SUMMARYWheeler Clinic’s Child First program is seeking a Senior Counselor to visit with children, birth to age 5, and their caregivers in the family’s home. The Senior Counselor teams with a Clinician to help heal and protect children and their caregivers from the effects of trauma and chronic stress. The team strengthens caregiver-child relationships, supports caregivers to understand the meaning of their child’s behavior, and connects families to community supports. The Senior Counselor assesses family service needs and facilitates connections to community-based services. The Senior Counselor also engages caregiver and child in activities that stimulate the child’s development. Together the team focuses on enhancing the caregiver’s capacity to provide sensitive, age-appropriate care and protection, promoting security and mutual joy in caregiver-child relationships. The Senior Counselor will receive thorough training in the evidence-based Child First model, Circle of Security Parenting, and the Abecedarian Approach. Previous experience working with client population. Effective time management, strong verbal and written communication skills are required. Bilingual Spanish speaking candidate preferred.LOCATIONBristol is the office location. Will travel to Bristol, New Britain, CT and the surrounding northern towns. This position is remote; you can work from home and head out to your appointments and may need to stop into the office in Bristol at times.ESSENTIAL DUTIES AND RESPONSIBILITIESProvides data support using the Child First Comprehensive Record to help inform program managers and maintain program requirements.Participates in reflective supervision at the individual and team level with Program Manager and Child First teams.Develops case management plans or plans of care and provides direct case management services and/or coordination of care specific to each assigned client and/or family.Provides front line crisis support to individuals and/or families in the program by phone, in-home and/or in the community as needed.Makes decisions based on interpretation of the data presented and/or refines the methods and techniques used.Determines the processes and methods to follow based on assessment of the client/family presentation and their strengths.Provides group, individual and/or family counseling to clients with appropriate supervision and coordinates treatment phases.Maintains records of counseling, coordination of care, child and family team meetings and/or case management activities for reporting and tracking purposes.Demonstrates knowledge of basic psychopathology, crisis theory and other behavioral health concerns, and primary intervention modalities relevant to target population.Supports daily program specific operational functions as required.Provides service in the office and/or community settings outside of the office and maintains a flexible schedule.Participates in the development, implementation, and facilitation of initial treatment plan or plan of care.Demonstrates knowledge of internal and external levels of care, referral processes, and resources.Meets direct service productivity goals as defined by program and adheres to practice standards.Identifies potential risk factors and takes measures to reduce those risks in the home.Attends and is prepared for scheduled supervision, team meetings, staff meetings or rounds.Communicates in a positive, effective manner with client, family, and all internal and external providers.Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served.Wheeler Clinic is an equal opportunity employer.
