Conducting patient interviews and assessments, addressing physical and psychosocial health needs.Developing individualized care plans and setting patient outcomes based on assessment.Implementing and adjusting care plans to ensure safety and optimize patient outcomes.Administering prescribed treatments and medications, documenting patient responses.Collaborating with physicians and healthcare teams to coordinate care and treatments.Educating patients and families about conditions, medications, and post-hospital care.Assisting in discharge planning and ensuring patients receive the appropriate follow-up care.Skills You Need:
Current License to practice as a Registered Professional Nurse in New York State.Graduate from an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.Enrollment in a BSN program within two years and completion within five years of hire.Strong clinical judgment and leadership skills.Ability to work effectively in a fast-paced, team-oriented environment.
