Direct Supervisor

Building Principal, may receive assignments from designated personnel as required.

Essential Functions & Responsibilities

To perform this job successfully, an individual must be able to perform each essential function satisfactorily. The requirements listed are representative of the knowledge, skill, ability, physical demands and working conditions required of this position.

Demonstrate support of the Oak Lawn Hometown School District #123 strategic plan.Uphold the established Board of Education policies, regulations, and administrative proceduresDirect Health Care ServicesProvide limited direct health care/first aid to students; provides emergency care as necessary under the direction of the nurse; identifies and reports suspected victims of child abuse to the appropriate officials and agencies.Refers student requiring medication administration and/or diabetic management to nurse on site; request direction from administration regarding these student needs if nurse is unavailable /not on site.Identifies recurrent health complaints and refers to nurse. Assists nurse with vision and hearing screenings.Performs head lice screenings as necessary.

SafetyMaintains and updates medical emergency forms/computer information; assists the nurse in maintaining a list of students with serious health problems.Assists with, practices and participates in the implementation of universal precautions.Demonstrates knowledge of emergency procedures.Orders necessary health office supplies as needed.

Data Collection and ReportingMaintains current and accurate health records including assisting with documentation of student visits; alerts nurse to any serious health problems. Vision and hearing screenings; prepares log sheets for state mandated populations requiring vision and hearing screenings, audits screening results to prepare and maintain accurate rescreening lists, distributes referral paperwork to parent/guardian, maintains accurate records regarding referrals sent/returned, alerts nurses to referral paperwork not returned, sends follow-up paperwork/makes phone calls for referral paperwork not returned, and prepares data or vision and hearing reports for nurse review/submission.Immunization reports: records state required immunization information on/in pupil records, reviews immunization records for necessary follow-up, notifies parent/guardian of immunization deficiencies, alerts nurses to existing immunization non-compliance and prepares data for immunization report for nurse review/submission.Dental and vision exams: collects required dental and vision examinations as required by state regulation, reviews dental and vision records for necessary follow-up, notifies parent/guardian of dental vision exam non-compliance, alerts nurse to existing dental and vision exam non-compliance and prepares data for dental and vision reports for nurse review/submissionAssists the nurse with data/list retrieval as requested

Communication and CertificationMaintains a high level of ethical behavior and confidentiality when dealing with students and staff information.Communicates effectively with nurse, staff and parents/guardians regarding health care needs; identifies self as School Health Assistant when communicating with staff and parents/guardians.Participates in health personnel meetings as appropriate.Maintains certifications as required; adheres to IDPH, State and District policies and procedures.Performs other tasks as directed by the administration.

Communicate and cooperate with colleagues, students, parents, and the community as needed to meet professional standardsMaintain accurate records (students' performance data, clerical responsibilities, Medicare billing, etc.) for the purpose of documenting accurate student information, reports, and Medicare reimbursementTrack student attendance and look for trends in communicable diseasesMaintain and improve professional competenceParticipate in meetings and perform other responsibilities as assigned by the principal or designee in accordance with the Board of EducationAbility to sit/stand for prolonged periods of timeRegular attendance and/or physical presence at the job

Evaluation

Performance in this position will be evaluated by the Building Principal or designee in accordance with the Board of Education's policy on Evaluation of Professional Personnel.
