OT Experience - Required Mostly Outpatient w/ Acute Care Pediatric Experience - Preferred Please notate if your traveler has EPIC experience in the last year Profile Requirements: Work History Skills Checklist, current to 1 year, pertinent to the specialty 2 Supervisory Reference Forms, taken within the last two years Licensure (MI LARA) & Certifications (BLS - AHA) Candidates' Permanent Address (Facility will not consider local candidates - 50 mile radius restriction) CERTIFICATION REQUIREMENTS BLS (AHA) STATE LICENSE REQUIREMENTS Michigan
