Overview:
Under the direction of the Assistant Director of Nursing, responsible for ensuring the proper delivery of nursing services in compliance with OPWDD and DOH regulation. Practices and promotes the agencys mission statement in every interaction.Responsibilities:Practices and promotes Citizens mission statement in every interaction.Through family and community engagement, Citizens supports People to live the life they chooseProvides hands on clinical support to people supported including but not limited to assessments, medication administration, responding to emergencies, nursing skills within scope of practice. Provides clinical direction to LPN & AMAP staff and ensure that nursing skills are be performed to standards.Ensures the implementation of proper principles of primary nursing, in accordance with OPWDD and DOH regulation, ensuring that all peoples health needs are met in the home in which they are assigned.Is on call to the program on a rotating 24/7 to assist with triage of medical concerns when an RN isnt immediately on site.Completes a plan of nursing services (PONS) for each person supported on their caseload and ensures that all required documents are thorough, complete and up to date (i.e. CFA, PONS, Braden scale) Initiate and complete training every time there is a change in a persons status and no less than annually.Ensures that all consultations, lab work, medical visits, medications etc are completed as ordered for a designated caseload.Participate in all nursing staff meetings & attend team meetings, administrative and management meetings to communicate medical information as needed and seek recommendations to improve medical care to individuals in each program.Ensures that nursing department adherence to all ICF regulations and participates in ongoing surveys by OPWDDConfers with individuals who receive supports and services and their family members/correspondents by telephone and scheduled treatment team meetings to serve as an insightful resource and to discuss their specific questions related to their medical concerns.Ensure that work-related safety procedures are in place and are being adhered to by staff as it relates to workplace safety and infection control.This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check.Reports to Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect or a potentially hazardous work environment. Additionally, follows all agency incident policy and procedures.Adheres to all regulations, agency policy and procedures, the employee handbook, and agency and program training requirements.Performs other duties as instructed by supervisor.
Qualifications:NYS Registered Nurse License, 1 year experience preferred but not required
