The nurse will collaborate with a multidisciplinary team within Duke Health to include Social Services, HIV Rapid Response Reengagement team (H3RT), as well as Duke Well in order to coordinate care for HIV patients that are out of care (OOC).

This role will also assist in collecting data on barriers to HIV care access within Duke Health. Following and completing phone calls to OOC patients ensuring that they have follow up schedules with an Infectious Disease provider as well as perform other related duties incidental to the work described above.

Duties and Responsibilities of this Level

Plan and provide professional nursing care for patients in accordance with physician orders and established policies and procedures to include collecting and analyzing patient data, developing and implementing nursing care plans, establishing interpersonal relationships with patients, evaluating nursing care given and communicating to proper personnel relevant facts about patients, action taken and patient responses.Assist in the development and implementation of general policies and procedures designed to effect efficiency of operations.Education

Work requires graduation from an accredited BSN program.

Experience

Twelve months of appropriate clinical experience is required.Preferred 3-4 years of experiencePreferred Infectious Disease and Care Coordination experience.Degrees, Licensure, and/or Certification

Must have current or compact RN licensure in the state of North Carolina or Compact license from participating state.BLS required, but may be updated during orientation as need be.Knowledge, Skills, and Abilities
