Position Summary:

Eventus WholeHealth delivers an integrated model of care to medically vulnerable adults across multiple states through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and In-house Support Staff. The position will ultimately uphold the mission to provide holistic care for medically vulnerable people, using the vision of an integrated model of care that provides extraordinary outcomes for the people we serve. This will be achieved using our values of excellence, integrity, person-centeredness, stewardship, and teamwork. The State Chief Medical Officer will be the key clinical leader of medical services across each State served by Eventus WholeHealth.

Essential Duties and Responsibilities:

The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive, other duties may be assigned:

Clinical responsibilities:

Assist with physician coverage, when needed. Follow the job description for “clinical role.”

Lead the training of new physicians in collaboration with the training/onboarding team.

Be available for consultation to all medical physicians and clinicians for clinical guidance and recommendations on clinical standards in all settings.

Leadership responsibilities:

Engage with your physician team on a set frequency, no less than monthly, to ensure the following:

Ensure Physicians are executing on the Eventus clinical model of care.

Identify barriers that affect these medically necessary visits.

Ensure physicians are empowering, motivating, and mentoring the advanced practice providers whom they supervise. They should be meeting with them no less than monthly and interacting with them more frequently on patient care cases.

Reinforce the integrated model of care and promote communication amongst disciplines. Physicians should be reinforcing the collaboration between the PC team and the MH team.

Lead clinical compliance and quality for the state. Coordinate with Auditing & Monitoring and Compliance teams on related initiatives.

Identify any barriers that affect physicians’ ability to have timely note turnaround and create solutions to eliminate this barrier.

Identify other potential leaders among the group and harness that interest.

Identify and take appropriate action on any novel thoughts, ideas, possibilities that advance our mission and vision.

Identify those that would meaningfully contribute to certain committees in this organization.

Ensure the principles and requirement of our value-based care initiatives are being enacted/executed.

Be responsible for management of physician performance (to include but not limited to: compliance, quality, customer service, productivity, PTO coverage, adherence to the clinical model, and VBC initiatives).

Work with the education department on training and ongoing educational needs for the state and Eventus as a whole. This includes writing and delivering at various seminars, following up on educational gaps, and contributing to quality committee.

Work with the recruiting team to hire new physicians in your state.

Work with business development on securing new facility contracts across SNF/ALF care settings across the state.

Advance strategic relationships with outside medical entities such as hospitals, other healthcare providers/HHC/Hospice, community primary care/specialty groups, and payors.

Advance strategic relationships with SNF/ALF owner-operators in your state.

Advance strategic representation at national trade organizations such as PALTC-AMDA.

Engage with other State CMOs to work together on the above to problem-solve and create innovative solutions together. Interaction with each other advised at least monthly.

Engage with Associate CMO-PC and Associate CMO-MH on a frequency, no less than bimonthly for review of the above. This meeting will be for all state CMOs together.

Clinical role:

Clinical/Quality Expectations:

Provide medical services in accordance with Eventus WholeHealth’s specific protocols, policies, best practices, quality care initiatives and consistent with the standard of care for the specialty of internal and geriatric medicine as it pertains to the patients served.

Diagnose and treat conditions and initiate referrals and consultations in accordance with established standards of care.

Interview patients to obtain health history and perform physical and/or psychiatric exams and diagnostic procedures as applicable.

Order, interpret and evaluate tests to identify patients’ clinical problems and health needs.

Record physical and/or psychiatric findings and develop care plans.

Formulate prognosis based upon patient’s condition and clinical findings.

Counsel patient and/or surrogate decision makers in health care management.

Prescribe medication and treatments in accordance with protocols, practice agreement and prescriptive authority.

Ensure patient information, visit history, referrals, lab follow-up, phone calls, and consults are carefully and completely documented in the EMR.

Complete documentation and submit billing records to Eventus Whole Health according to published policies.

Review and sign documents as requested by facilities and specific to assigned duties as a provider while at the facility or within 24 hours of receiving such request by fax or email.

Review and address labs within 24 hours of receipt.

Education and/or Certifications:

Medical physician with board certification in Internal Medicine or Family Practice.

Unrestricted license to practice in own specialty in the state(s) for which the position is located.

Skills and Qualifications:

Experience in long term care x 5 years.

Strong leadership skills and the ability to effectively coach, motivate and develop others.

Excellent oral, written and interpersonal communication skills with the ability to bring individuals together.

Maintain exceptional verbal, written and electronic communication skills along with the ability to work collaboratively and coordinate care with a multi-disciplinary team.

Possess outstanding organization skills and an ability to multitask while maintaining a customer focus.

Demonstrates creativity, problem-solving adaptability and flexibility.

Computer proficiency in MS Office, electronic health record systems and other business technology tools.

Ensure compliance with all federal, state, and local laws, regulations, standards, guidelines, and Eventus WholeHealth policies and procedures pertaining to the job.

Remain informed of any changes to federal, state, and local laws, regulations, standards, guidelines, and Eventus WholeHealth policies and procedures that impact the job and the company.

Promptly report any non-compliant conduct and/or practices to Eventus WholeHealth’s Chief Compliance and Risk Officer.

Ethical Conduct:

Ensure adherence to Eventus WholeHealth's Code of Ethics and Business Conduct.

Perform the job duties with integrity and professionalism.

Promptly report any unethical conduct and/or practices to Eventus WholeHealth’s Chief Compliance and Risk Officer.

Patient Privacy and Confidentiality:

Strictly protect patient information in accordance with all federal and state privacy laws and Eventus WholeHealth policies and procedures.

Promptly report any violations of federal and state privacy laws and Eventus WholeHealth policies and procedures to Eventus WholeHealth’s Chief Compliance and Risk Officer.

Documentation and Record Keeping:

Maintain accurate and complete documentation when performing the job duties.

Ensure that all Eventus WholeHealth information, data, documentation, and records are always safeguarded and maintained confidentially.

Training and Education:

Participate in all assigned training.

Complete all assigned training by the deadline for completion.

Risk Management and Incident Reporting:

Engage in regular risk assessments to identify potential risks and work proactively with a supervisor and Eventus WholeHealth’s Chief Compliance and Risk Officer to mitigate any identified risks.

Promptly report any incidents (as defined in the “Incident Report Policy”) to the Chief Compliance and Risk Officer.
