The Director of Population Health reports to the Chief Medical Officer and is responsible for the improvement of health outcomes, the quality of care, and value of care for the population of patients served by Baltimore Medical System.

Responsibilities:

Manages day-to-day activities to improve population health outcomes, quality of care and value of care to Baltimore Medical System’s patients.

Works with the Executive team and the Clinical Leadership team to develop, implement and evaluate population health, quality, and practice transformation initiatives and strategies.

Uses improvement methodologies (i.e. PDSA, Lean) to lead teams in quality improvement.

Collaborates with payers, hospitals, funding agencies, accrediting organizations, and community partners to improve the performance of Baltimore Medical System on quality, utilization and other performance measures.

Manages and coordinates value-based purchasing agreements from payers. Communicates requirements for Leadership and the Clinical Teams.

Works with the Chief Medical Officer to design and implement data driven programs that assist in the identification and stratification of high-risk populations.

Works with the Chief Medical Officer to identify gaps in the quality of care and assists in the development and implementation of education, and training/technical assistance programs for Baltimore Medical System leadership and staff.

Understands requirements of the Joint Commission, HRSA, National Committee of Quality Assurance (NCQA) and assists in ensuring compliance with the standards involving population health and quality improvement.

Provides recommendations to align quality and performance measures from payers (HEDIS), UDS, Patient Centered Medical Home (Joint Commission and NCQA) in order to streamline improvement activities.

Interfaces with Health Informatics and Information Technology staff to develop systems to collect data for reporting.

Oversees quality improvement teams and makes recommendations for activities and improvement cycles.

Participates in local, state and national population health and quality improvement meetings as directed by the Chief Executive Officer or the Chief Medical Officer.

Provides presentations to Baltimore Medical System staff, Board of Directors and external partners regarding plans, progress, and status of population health and quality initiatives.

Provides monthly reports on progress with quality and performance improvement initiatives at Baltimore Medical System.

Assists the Chief Medical Officer in data display (i.e. dashboards, scorecards) of improvement measures for Leadership, clinicians and staff.

Qualifications:

Bachelors or Masters degree in healthcare field.

A minimum of 3 years of experience in healthcare management, quality/performance improvement, public health or related field and a minimum of one year of supervisory experience.

Must be familiar with data analytics for population health and have strong computer and presentation skills.
