Health and Wellness Director/LPN

Job OverviewWe are seeking a dedicated Wellness Director to join our team. The ideal candidate will oversee and coordinate all wellness programs to promote health and well-being within our organization.Responsibilities:Interview residents and families prior to admission and coordinate pre-admission screening and assessment.Evaluate resident care needs and determine the required level of care upon admission and changes in condition.Update medical charts and care plan.Order medications from pharmacy, maintain records and ensure that medications are properly stored and cared for.Maintain records and files for residents, inventory, and supplies.Monitor associates in administration of medication, including frequent audits of medication records.Skills:Must be a LPN in the state of practiceMust have a Bachelor’s degree with emphasis in business administration, hospitality, gerontology, or another related field.This position also requires 1-2 years of supervisory experience, prior care experience in an assisted living community or skilled nursing facility, and valid First Aid and CPR Certification.Why join us?
