Job Summary:
We are seeking a dedicated and experienced Primary Care Nurse Practitioner (NP) to join our team. The ideal candidate will have experience in addiction medicine and be proficient in providing care in both residential and outpatient settings. The NP will conduct comprehensive History and Physical exams (H & P's), deliver basic primary care services, manage withdrawal symptoms, and treat both adults and adolescents with substance use disorders in person and virtually via telehealth.
Key Responsibilities:
Conduct comprehensive History and Physical exams (H & P's) for new and existing patients. Provide basic primary care services, including diagnosis, treatment, and management of acute and chronic conditions. Manage withdrawal symptoms and provide medication-assisted treatment (MAT) as needed. Develop and implement individualized treatment plans in collaboration with a multidisciplinary team. Provide care to both adults and adolescents in residential and outpatient settings. Educate patients and their families about substance use disorders, treatment options, and recovery strategies. Monitor patient progress and adjust treatment plans as necessary. Maintain accurate and timely documentation in electronic health records (EHR). Participate in team meetings, case conferences, and continuing education activities. Adhere to all regulatory and accreditation standards.
Qualifications:
Master's or Doctorate degree in Nursing from an accredited institution. Current and unrestricted Nurse Practitioner license in the state of Illinois. Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP). Minimum of 2 years of clinical experience in primary care. Experience in both residential and outpatient care settings is preferred. Certification in Addiction Nursing (CARN) is a plus. Strong clinical assessment and diagnostic skills. Excellent communication and interpersonal skills. Ability to work collaboratively within a multidisciplinary team. Commitment to providing high-quality, compassionate care to individuals with mental illness and substance use disorders.
