Home Care Agency in need of a PART-TIME Physical Therapist that will cover Alameda County.
At Professional Home Care Associates, we provide outstanding Home Health Care to each patient in the privacy and comfort of their home. We are a leader in developing and implementing effective diagnosis specific to home care protocols, and we rely on you to make key decisions about your patients care. Professional Home Care Associates is owned by two Physical Therapists, making patient care our priority.JCI Accredited as of April 2014*Essential Functions:** Accepts assignments and performs care as described in coordination with physician.* Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional abilities tests.* Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means.* Observe, record and report to the physician and/or supervisor the patient’s reaction to treatment and any changes in the patient’s condition.* Instruct patient in care and use of wheelchairs, braces, canes, crutches and prosthetic and/or orthotic devices.* Instruct other health team personnel, including home health aides and the patient’s family, in certain phases of physical therapy with which they may work with the patient.* Instruct family on patient’s total physical therapy program.* Prepare evaluations, progress notes, summaries and all communication of patient progress as directed by supervisor and agency policy.* Participate in in-service education, utilization review and team conferences.* Supervise Physical Therapy Assistants as outlined by the California Code of Physical Therapy regulations.* Review, co-sign and date all documentation by the Physical Therapy Assistant or conduct a weekly case conference and document in the patient’s record.* Reports changes in the patient’s condition and needs to the case manager, to the Supervisor and/or the physician.* Completes records appropriately.* Turns paperwork into the office in a timely manner*EMPLOYMENT STANDARDS:*
* Must have Physical Therapy License form the State of California* Own transportation, Drivers License and Automobile Insurance* Excellent oral and written communication skills* Strong Physical Therapy skills* Commitment to practice quality service delivery* Attends 12 hours of in-service annually

Pay: $80.00 - $120.00 per hour
