PURPOSE OF THIS POSITION

The purpose of this position is to develop, organize, implement and direct the day to day activities of a patient care unit in accordance with Joint Commission, Federal, State and Local guidelines as well as our established policies and procedures.

JOB DUTIES/RESPONSIBILITIES

  • Duty 1: Directs activities in compliance with regulatory agency requirements (Joint Commission, Ohio Department of Health, etc.) Stays abreast of changes and responds in a timely manner. Complies with organizational policies, procedures, and practices to ensure quality job performance and Medicare/Medicaid compliance.
  • Duty 2: Demonstrates the knowledge and skill to mentor staff in providing consistent quality patient care to those being referred to or receiving Bridge services.
  • Duty 3: Works with the HH Therapy Manager in managing the overall operations of the HH team, including staffing, scheduling, payroll, departmental policies and procedures, associate/physician relationships, and departmental assets.
  • Duty 4: Assists in the selection and training of competent department associates in conjunction with Human Resources. Establishes, participates, and evaluates orientation and ongoing department competency requirements and education for staff.
  • Duty 5: Identifies opportunities for process improvement. Directs, implements and evaluates unit based on process improvement efforts. Works collaboratively with other Bridge services to achieve process improvement initiatives.
  • Duty 6: Supervises the staff in performance of their daily duties and assisting staff with assessment/intervention/evaluation skills.
  • Duty 7: Facilitates Interdisciplinary Team meetings and communication/collaboration with the other Bridge Teams.
  • Duty 8: Participates in On-Call duties as assigned.
  • Duty 9: Ensures staff’s documentation is consistent with or exceeds Bridge Home Health and Hospice guidelines.
  • Duty 10: Displays Service Excellence as evidenced by practicing the mission, vision, and values of the organization to promote patient satisfaction.

REQUIRED QUALIFICATIONS

  • Licensed by the Ohio Board of Nursing as a Registered Nurse
  • Two years RN experience preferably in a Home Health or Inpatient setting
  • Performs duties of direct patient care RN as needed
  • A valid driver's license is required (if you do not have a valid Ohio driver’s license you must obtain one within 30 days of your residency in the state). You must also meet BVHS's company fleet policy and insurance company requirements, and any other requirements that may be required to operate a vehicle.
  • Positive service-oriented interpersonal and communication skills required.
  • Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status. Must be able to interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs and to provide the care needed as described in the area’s policies and procedures

PREFERRED QUALIFICATIONS

  • BS/BSN
  • Supervisory experience
  • Demonstrated management skills

PHYSICAL DEMANDS

This position requires a full range of body motions with intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The associate will be required to walk for up to one hour a day, sit for four to six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel and community involvement.
