Manager, Social Work . The Manager, Social Work is responsible for the leadership, direction, productivity, and clinical performance of the Medical Social Work Department for all Cottage Health facilities. Responsibilities include but are not limited to:Discharge planning and psych-social support.Ensures appropriate medical social work is based on protocols, evidence-based guidelines, physician plan of care and best practices to assure optimum medical outcomes.Partners with Case Management to ensure effective transition to the next level of care.Utilizes metrics to facilitate effective decision making.Manages with a 'Patients First' orientation.Fosters and maintains strong physician relationships.Continually cultivates and supports strong interdisciplinary teamwork.Ensures individual team member training, competency, and professional skill set development.Engages and promotes positive communication and interdisciplinary interaction.Successfully achieves departmental and organizational goals/objectives.QUALIFICATIONSAll job qualifications listed indicate the minimum level necessary to perform this job proficiently.LEVEL OF EDUCATIONMinimum: MSW degree from an accredited School of Social Work with field placement or job experience in hospital, health, or mental health or related setting.Preferred: MHA or similar.CERTIFICATIONS, LICENSES, REGISTRATIONSMinimum: LCSW (CA State Licensed Clinical Social Worker).TECHNICAL REQUIREMENTSMinimum: Proficient in Microsoft Office Suite.YEARS OF RELATED WORK EXPERIENCEMinimum: Five years experience in a healthcare setting with 2 years administrative, supervisory, or consultative responsibility in a social work agency.Preferred: Greater than 5 years experience providing medical social work services in an acute care setting, and greater than 2 years of supervisory experience of medical social workers in an acute care setting.About UsCottage Health is a leading acute care hospital system, located on the central coast of California, widely known for our superior patient care, innovation, medical research, and education.
