Job Description:Grow Medicare and overall Census through existing pathways and relationships while developing new and innovative census growth initiatives.Innovative alignment, affiliation, development, and operationalization of value-based care (VBC) and alternative payment models (APMs), methods, programs, and care re-design.Strategic focus including Accountable Care Organization (ACO) engagement, managed care relationship enhancement, clinical integration network development, value-based payment model adoption (episodic payments/ bundled payments), and forward-thinking continuum connectivity.Focus on facility and market Key Performance Outcomes (KPIs), narrow network selection/affiliation status, preferred provider relationships, by leveraging and actualizing facility-based leaders and teams.Active engagement and relationship development with continuum partners, including Hospitals/Healthcare Systems, Health Plans, IRF, LTACH, SNF, HHA, ALF, Hospice, Urgent Care, Mobile Diagnostics, Laboratory, DME, Pharmacy, Physicians, Physician Groups, Managed Care Organizations, Non-emergent medical transportation, Community Para-medicine Programs, etc.Create in-roads and establish a "seat at the table" for EDs (and their teams) with Hospitals, Systems, Networks, Conveners, Providers, and other value-add continuum and referral partnersProvide direct resource support, on-boarding, and education to Facility Marketers and Liaisons. Support, On-board, and Educate Admissions Team Members. Facilitate and support patient intake and placement opportunities. Support Case Management functions.Optimize and operationalize managed care contracts and census-related opportunitiesProvide analytic support and data on key revenue, census, market share, competitor comparatives, and profitability metricsSupport pre-acquisition assessment/analysis, due-diligence, and acquisition activitiesManage, negotiate, and support Value-Based FFS/CMS Program Contracting and ParticipationKey Performance Indicators:

ADC, Occupancy, Skilled Mix, Revenue PPD, ALOS, facility profitability (EBIT), new/not-yet-captured revenue, and reduction of "Lost Opportunity Revenue"Quality Outcomes:

Strong analytical, data, and financial skills. Highly proficient in Excel, Data Analytics, Pivot Tables, Report Generation, etc.Strong leadership and communication skills.Bachelor's degree and/or master's degree in healthcare or business-related field (preferred)Active and current knowledge of the post-acute care continuum, health care policy and alternative payment models (BPCI, ACO, DCE, Managed Care)Minimum of 3 years' experience within the post-acute setting (preferably SNF) and at least 2 years of leadership experience within a healthcare setting.Experienced in relationship building and establishing trusting and lasting associations with continuum providers.Must be highly professional in demeanor and have strong presentation abilities for presenting to large groups of healthcare providers, executives, and peer groups.Must be a team player, critical and creative thinker, and able to build relationships across the team in order to push PACC initiatives forward.Preferences:
Experience leading and influencing teams in Skilled NursingExperience as a Licensed Nursing Home Administrator, clinician, or therapist (preferred, but not required)Cross-continuum leadership and experience, including acute care/hospital tenureMaster's level education (preferred, but not required)Entrepreneurial drive and an ability to live Ensign cultural tenets including Celebration, Accountability, Passion for Learning, Love one Another, Intelligent Risk-Taking, Customer Second, and Ownership (CAPLICO).

Salary: $100,000-$135,000/variable with experience may be eligible for bonus
