Establishes discharge plans with patient, physician, other disciplines and caregivers.Completes OASIS dataProvides patient care using current Federal, State and Agency regulationsAssesses educational needs of patient, families, caregivers and implements appropriate teaching strategiesCompletes documentation according to regulatory guidelines.Promotes a clean and safe environment of care, utilizing the SAFE error prevention habitsProvides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct

REQUIREDValid drivers license and car insurance -
REQUIREDCopy of agency's IL nursing license included in submission -
REQUIREDWill consider someone with strong Med Surg experience with some Home Health experienceCOVID and FLU vaccination required - DECLINATIONS WILL BE APPROVED
