Job SummaryThe Director of Care Redesign – East Baltimore reports to the Vice President, Medical Affairs at The Johns Hopkins Hospital (JHH), with a dotted line to the Executive Director, Care Management & Social Work at Johns Hopkins Bayview Medical Center (JHBMC).
This position spans the academic campuses in the Baltimore-metro area and is responsible for the development and implementation of initiatives designed to reduce readmissions, avoid preventable utilization, and improve equitable access to care through the deployment of evidence-based care delivery models and programs. The director identifies JHH/JHBMC care delivery opportunities/gaps and develops creative solutions to address them – especially within the context of the state’s Care Redesign efforts.The director will represent JHH and JHBMC at appropriate HSCRC, MHA, BCHD and other applicable forums and will serve on planning and work groups related to health system, city, and state initiatives.
The individual in this position must be knowledgeable in the areas of health care delivery systems, social determinants of health and health care transformation, and understand relevant Federal and State regulatory requirements, including the Maryland All Payer Model.Qualifications:Master’s degree or higher in business administration, health economics, health care administration, public health, or closely related field strongly preferred.Minimum of 7 years progressively responsible professional level administrative management/leadership and financial experience in care coordination, clinical resource management, population health and care delivery systems.Ideal candidate has experience working with health systems, hospitals, physician groups, and community-based organizations.Experience working with and collaborating successfully with senior hospital and community leaders, and experience in preparing and presenting information to leadership.Population Health experience strongly preferred.
