Position Summary: The Case Management Director develops and integrates strategies to enhance the quality of care for clients. In addition, the Case Management Director stays up to date with government policies and procedures and ensures clients are discharged in a safe and timely manner in accordance with ORR guidelines.
Roles & Responsibilities:
Provide ongoing administrative oversight and case management supervision to foster a solution and strengths-based orientation to assist case management staff in effectively addressing the child’s ongoing service needs and demonstrate a positive, strength-based attitude of teamwork and shared leadership.
Provide ongoing support and recommendations for facility improvements and upgrades as needed.
Act as the responsible party for the overall work and performance of the case management department.
Prepare and review required reports including Quality Management Reports, Case Worthy Performance Reports, and other UC Portal mandated reporting.
Support the development and execution of training activities related to Integrated Health Care and High Needs Case Management.
Monitor the High Needs Case Management/Intensive Care Coordination model to ensure the case load to staffing ratio is adequate.
Oversee implementation of all contract PWS requirements.
Collaborate with the Director of Behavioral Services to ensure adherence to quality standards by providing clear and consistent application of ORR policies, ensuring proper documentation of services, and modeling the highest ethical, legal and client care standards.
Oversee the submission of daily Office of Refugee Resettlement required reports.
Monitor and evaluate program activities, including client referrals, intakes, case strategy, and case closures.
Identify training needs and program challenges, and lead the development of materials, resources, and tools to enhance the delivery of program services.
Basic Qualifications:
Bachelor’s degree in social services, human services, or related field.
Minimum of (5) five years case management experience, preferably in an ORR setting.
Minimum of (2) two years supervisory experience.
Must be computer literate.
Must be at least 21 years of age at the time of hire.
Proficiency in English and Spanish (verbal and written) preferred.
Possess a valid Driver’s License.
Possess a clear Tuberculosis test.
Ability to successfully clear a background check.
Ability to successfully clear a drug screening.
Provide immunization documentation for Tdap, Varicella, MMR, Hepatitis A and B, Varicella.
Must be authorized to work in the U.S. for any employer.
