Jersey City Medical Center, 355 Grand Street, Jersey City, NJ 07302Job Overview:Responsible for providing individual counseling services to Center for Comprehensive Care patients.Qualifications:Required:Masters Degree in mental health fieldLSW or LCSW RequiredExperience and knowledge of HIV and community resources is requiredPreferred:Bilingual strongly preferredScheduling Requirements:9:00 am – 5:00 pmMonday thru FridayFull TimeEssential Functions:Provides individual counseling services to CCC patientsConducts a comprehensive initial assessment on patientDevelops and implements individualized plan of care using short-term and long-term counseling plans for patientsConsults with Psychiatrist and other providers regarding complex patients and regarding prescription medications prescribed by providersProvides non-scheduled urgent assessment of patients with medical clinic appointmentsAssesses patients with urgent mental health conditions (e.g. suicidal, homicidal, psychotic, etc.) and makes appropriate referrals to available emergency or psychiatric treatment facilitiesProvides therapeutic interventions including crisis counseling to individuals and groupsProvides education to staff on mental health issuesRefers patients to necessary social and medical servicesDevelops, implements, and maintains partnerships with other community providersResponsible for maintaining up-to-date patient records and documenting each patient encounterResponsible for submitting and achieving target for monthly statisticsAttends and actively participates in weekly clinical supervision with supervisorComplies with unit and department scheduling guidelinesMaintains and updates professional knowledge and proficiency through continuing education, staff meetings, workshops, and serving on special committeesProvides task supervision and/or field instruction to master-level social work studentsResponsible for grant reporting and data tracking pertaining to mental health outcomesResponsible for attending all grant meetings pertaining to the mental health program on-site and off-sitePerforms other duties as assignedBenefits and Perks:At RWJBarnabas Health, our market-competitive Total Rewards package provides comprehensive benefits and resources to support our employees' physical, emotional, social, and financial health.Paid Time Off (PTO)Medical and Prescription Drug InsuranceDental and Vision InsuranceRetirement PlansShort & Long Term DisabilityLife & Accidental Death InsuranceTuition ReimbursementHealth Care/Dependent Care Flexible Spending AccountsWellness ProgramsVoluntary Benefits (e.g., Pet Insurance)Discounts Through our Partners such as NJ Devils, NJ PAC, Verizon, and more!Pay Range:$29.00 minimum to $31.00 maximum
