POSITION TITLE : Intake Counselor

LOCATION DEPT : Harbor Light

REPORTS TO : Intake Coordinator

DEPARTMENT MISSIONTo provide each client with positive, professional and quality service; always promoting respect dignity and fairness to each client; while making appropriate assessments and referrals which will assist clients in reaching their maximum level of independence.

PROGRAM/CLIENT ENVIRONMENTThe backgrounds of the clients served by The Harbor Light Program will manifest a variety of inappropriate behaviors. When this occurs, employees are to respond within the context of the treatment environment of the site. Employee conduct (actions, dress, etc.) and interactions (conversations, written communications, etc.) with clients must also be within the treatment environment of the facility.

OUTCOMESEvaluates clients for admission on the Harbor Light programs under established criteria of admission; presents cases with recommendations to the Review Board, operates under the supervision of the Intake Supervisor. Provides limited direct service via individual and group counseling.

ESSENTIAL DUTIES AND RESPONSIBILITIESTelephone calls and walk-in clients are screened for appropriateness for further evaluation based on admission policies, programs, and procedures; appointments are issued for evaluations; clients are evaluated and arrangements are made for medical screenings and psychiatric evaluations.Cases are presented for admission to the Review Board, decisions made by the Review Board are implemented.Clients who are at risk due to a physical or mental condition are admitted to the Transitional Living Unit (TLU) when approved by the Clinic Supervisor; IV drug users are admitted to TLU on a priority basis; clients who are in transition from one level of care to another are admitted to TLU.Client records and other related papers are sent to other departments as required in a timely manner.Clients from TLU to other Units are transferred when beds become available, clients are discharged from TLU when approved by the Review Board or due to relapses or violent behavior.A good working relationship is maintained with referring agencies and V.A. hospitals, services, policies and procedures are explained in response to inquiries from the community or other social service agencies.Clinic's staff committees and staff meetings are attended.Seminars, conferences and other meetings are attended when requested.Records are kept and statistical and other required reports are prepared.Clients are seen for individual and group counseling in OPC and IRU as assigned by supervisor.All other duties as assigned.

REPORTING RELATIONSHIPSThis position reports to the Intake Coordinator. In contacts related to this position's duties, this individual acts as a representative of The Salvation Army and its mission.

PERFORMANCE MEASUREMENTSThis individual will be evaluated on how effectively and efficiently the outcomes of this position are achieved, as well as the timeliness, accuracy and completeness of accomplishing assigned goals.

EDUCATION/EXPERIENCECertified in the State of Illinois as a Certified Addictions Counselor, and a minimum of two years of work experience. If not certified, able to obtain certification within 2 years of hire.A Bachelor's degree preferred.A minimum of two years of sobriety needed.

POSITION LIMITATIONSThis individual will only commit Army resources that have been allocated or approved.This individual will keep the Intake Coordinator informed on all critical issues relating to his/her area of responsibility.This individual will adhere to all Army policies and procedures in carrying out the responsibilities of this position.

PHYSICAL DEMANDS/WORK ENVIRONMENTThis position is required to do light physical work and must be able to walk up two flights of stairs and lift up to 25lbs.In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and PC.The work environment for this position includes an office environment with a medium noise level.Must be able to work with clients that have poor social skills, suffer from mental illness, and have substance abuse problems.
