We are seeking a dedicated and compassionate Psychiatric Nurse Practitioner to join our healthcare team, providing specialized mental health services to residents in senior living facilities. The Psychiatric Nurse Practitioner will be responsible for rounding on patients in nursing homes, assisted living facilities, and other senior living communities, offering psychiatric evaluations, medication management, and therapeutic support. The ideal candidate will be passionate about geriatric mental health and committed to improving the quality of life for older adults. The incoming provider will commit to the highest quality of care using an organized, multidisciplinary team approach to provide continuity of care throughout the post-acute continuum. Our patient population served includes adults and geriatrics.
Duties:Provide on-site psychiatric assessments and medication management to patients in skilled nursing, assisted living, and other long-term care facilities.Coordination of care with attendings, consults, referral sources, and staffPerform physical exams, obtain complete medical history, and record findingsOrder appropriate laboratory and diagnostic proceduresInterprets and integrates data to determine preliminary diagnosis and therapeutic planOrder/administer medication and injections as deemed medically necessaryInstructs patient/family regarding medications, post admission care and treatmentsEducates patients regarding health and illness preventionCollaborates with physicians in managing acute needs of patientsQualifications:
Graduation from an accredited Nurse Practitioner programANCC certification as a Psychiatric Mental Health Nurse PractitionerDEA licenseBLS & ACLS certifiedCurrent State license
