The Home Care Physical Therapist Assistant will provide physical therapy services to older adults within their home/community environmentCommunicate with Physical Therapists regarding plans of care and progression of exercises and goals for each patient.Maintain communication with Coordinator to successfully schedule your patients in a timely mannerComplete notes thoroughly and within a timely mannerAttend company-wide meetings to discuss various company initiatives to continuously improve clinical excellence and patient care.Medical Specialties
