Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We work every day for a fair and just society where all people have the opportunity to make choices that lead to better lives for themselves and their families.Program Description: Goddard Riverside's Assertive Community Treatment (ACT) Team provides psychiatric treatment and intensive case management for 68 adults with severe mental illnesses. Services include crisis intervention, medication monitoring, social supports, assistance with everyday living needs, access to medical care, psycho-educational supports, and employment assistance.Purpose of Position: The Registered Nurse delivers direct and comprehensive nursing for the ACT team clients, including overseeing and monitoring their medication management.Schedule: Flexible, 35 hours per weekSalary Range: $99,134.00 - $111,525.00Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate's number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.Roles, Responsibilities, and Essential Duties1) Medication managementCommunicate with team psychiatrist about client prescriptionsMonitor, dispense and document client medicationsAdminister injections2) Nursing servicesAssess psychiatric and medical status of clientsPerform medical assessmentsObtain vital statistics.Collect biological specimens Including blood and submit them to diagnostic laboratories.Interpret Lab resultsAct as liaison between ACT and pharmacy & Diagnostic LaboratoriesMaintain storage and documentation of onsite client prescriptions and medications3) Provide case management servicesCarry a small caseloadPerform client assessments and formulate treatment goalsFacilitate client participation in entitlement programs, psychiatric services, medication management, rehabilitation and other activitiesRefer clients to community health, substance abuse, psychiatric and other servicesAccompany clients to appointments as neededCommunicate regularly with clients' families about their needs and progressServe as liaison to other service providersMaintain up-to-date client case records according to program guidelinesVisit clients in residential facilities, on the street and in other locationsRotate on-call service after outside work hours4) Participate as an ACT team memberAttend staff meetings, case conferences, and training programs to upgrade skills and knowledgeAttend supervision meetings to review client progress and case management strategiesWork collaboratively with ACT team staff in a team case management model5) Other duties as requiredQualifications/Educational RequirementsActive NY State License as a Registered Nurse2- 3 years of nursing experience preferredExperience working with the seriously and persistently mentally ill populationDriver's License preferredSkills, Knowledge and AbilitiesAbility to speak Spanish, French, Creole, American Sign Language or other languages is a plusComputer Skills: To perform this job successfully, an individual should be:Able to work in the Microsoft Office Suite, should especially be proficient in Word and OutlookAble to use or learn to use AWARDS databaseAble to use or learn other electronic databases, programs, and funder-related software as neededPhysical RequirementsWalking; Climbing stairs occasionallyWork EnvironmentField & Office workWe are committed to building an inclusive workplace community grounded in respect for differences.
