We are looking for a highly motivated and energetic Medical Assistant / Care Coordinator to join our team as a Chronic Care Management (CCM) Onboarding Specialist!
In the role of a Chronic Care Management (CCM) Onboarding Specialist, you will serve as a key link between our patients and the essential services we provide. This role includes a strong focus on improving patient outcomes through effective communication, coordination of care, and compliance with healthcare regulations.
Primary Responsibilities:
- Patient Outreach and Enrollment: Actively promote our Chronic Care Management program to qualifying patients, clearly articulating the program benefits and securing their consent for enrollment.
- Patient Engagement: Maintain proactive communication with patients to address their queries regarding CCM services, and document all interactions diligently.
- Care Coordination: Execute onboarding calls, follow-up calls, medication checks with pharmacies, and liaison with healthcare providers to obtain relevant medical records.
- Healthcare Systems Navigation: Proficiently utilize various Electronic Medical Records (EMR) systems and adapt to the specific practices of various doctor offices.
- Patient Advocacy: Manage refill requests and other patient needs during and after onboarding calls.
- Compliance and Quality Assurance: Strict adherence to Health Insurance Portability and Accountability Act (HIPAA) guidelines and regulations.
Qualifications and Skills:
- Excellent verbal and written communication skills
- Chronic Care Management experience
- Familiarity with EMR systems
- Strong attention to detail and organizational skills
- Proficiency in time management and ability to multitask
- A solid understanding of HIPAA guidelines and regulations

Work Location: Enroll patients face to face in a Medical Practice across multiple Locations in Metro Detroit, MI . Mileage will be reimbursed.
