Family Health Center – Hawthorne, CADUTIES: Role and Responsibilities:

Direct all work activity of the Health Center Referrals Department.Plan, organize, and coordinate the operation of referrals for the Health Center Network.Provide oversight of daily operations for department including staffing, training, metrics, performance and reporting.Coordinate the implementation and maintenance of the electronic health record referral and tracking system, phone system, and other systems; act as a liaison with IT staff and work with staff to resolve any issues.Supervise, direct, train, and evaluate performance of referral staff across multiple clinical sites.Effective scheduling of staff and workflow to provide optimum coverage with available resources to meet department goals relating to production and quality.Answer inquiries, provide information, and resolve complaints from providers, patients, or other agencies regarding referrals.Review status on Treatment Authorization Request (TARs) and any other authorizations or requirements of managed care organizations to assure compliance with health plan programs.Follow-up on pending referrals.Cooperatively work with outside agencies to help patients apply for alternate resources when appropriate.Collaborate with clinical staff and the Quality Management department in reviewing, managing, and following up on tracking systems for immunizations and other outcome metrics.Investigate difficult cases and be the lead contact for patients’ or providers’ inquiries/concerns in obtaining appropriate care for the patient.Interview, hire, and perform disciplinary actions and performance evaluations for employees.Maintain inventory of supplies and equipment for the referral department.Ability to relate to the public regardless of racial, ethnic, economic, sexual orientation, and gender identity status.Maintain and foster relationships with community providers, agencies, and organizations involved in care transitions and continuity of care for patients.Work collaboratively as a team member with all staff members.Promote and believe in BHS mission statement.Other duties as assigned.QUALIFICATIONS:

Required Knowledge, Skills and Abilities:

Associate’s degree in healthcare or a related field.Possession of current California Driver’s License.Experience working for a community clinic and with Medi-Cal and Medicare.Thorough understanding of medical terminology, payor sources, insurance, and program requirements.Ability to build collaborative support systems with community agencies and organizations.Ability to demonstrate/convey respect for cultural and lifestyle diversities of clients and staff.Ability to present to diverse communities/populations, at all levels of internal/external organizations.Ability to recognize personal issues that may impact job performance and interactions with clients.Ability to note documentation in electronic medical record systems.Strong verbal and written communication skills.Proven track record of, and ability to demonstrate ethical and professional standards of behavior.Basic knowledge of referrals, both in and out of the BHS treatment system.Basic knowledge and experience in utilizing Microsoft Office, specifically MS Word and Excel.Able to speak, understand, and communicate in Spanish.Desired Knowledge, Skills and Abilities:

Knowledge of drug and alcohol dependency, mental health, family systems, and 12-Step/recovery resources in the community.Experience participating in Patient Centered Medical Home (PCMH) model of care.Physical Demands:

While performing the duties of this job, the employee is regularly required to stand, sit, talk, hear, and use hands and fingers to operate a computer and telephone keyboard.Light to moderate lifting is occasionally required (up to 25 lbs).Ability to sit at a computer terminal for an extended period of time.Ability to travel locally to clinical sites including, but not limited to: Santa Ana, East LA, Wilmington, Carson, and Hawthorne.Work Environment:
