Job Summary:We are seeking an experienced and dynamic Director of Hospital Case Management to lead and coordinate all departmental activities, ensuring compliance with organizational policies and regulatory requirements. Your leadership will be crucial in integrating patient care and discharge planning processes, optimizing clinical resource utilization, and ensuring continuity of care. You will oversee daily operations, engage interdisciplinary teams, and achieve established outcomes, all while maintaining a strong executive presence and excellent communication skills.

Key Responsibilities:

Direct and evaluate departmental operations, including staffing, information technologies, onboarding, and staff competencies.

Allocate resources to meet productivity and quality goals.

Oversee case management functions, ensuring adherence to care coordination and discharge planning processes.

Collaborate with the Facility CFO and Division CM Leader to achieve departmental goals.

Assess and improve department performance through data analysis and performance improvement activities.

Ensure compliance with organizational policies and standard operating procedures.

Establish working relationships with key stakeholders, including CMO, CNO, and ancillary service leaders.

Perform other duties as assigned and adhere to the "Code of Conduct" and "Mission and Value Statement."

Minimum Requirements:

Bachelor's degree in Nursing (required)

Master's degree in Nursing, Health Administration, or Business Administration (preferred)

3+ years of acute care hospital case management experience

2+ years in case management leadership in an acute care setting

Previous HCA experience highly preferred

Extensive leadership experience in case management is essential
