Women on the Rise is seeking a Reentry Specialist who will need to have some experience working with individuals that are returning from incarceration and/or have been directly impacted by the justice system. The Reentry Specialist under the direct supervision of the Re Entry Supervisor will provide client advocacy, care management, benefit establishment, linkage to Mental Health/Substance Abuse services, linkage to stable housing and all other supportive services as needed. Working under the supervision of the Executive Director, the Reentry Specialist will assist each participant in addressing any barriers identified in their initial assessment, stabilization of their income, and maintaining and sustaining mental health care, substance use abstinence, and/or permanent housing.

Responsibilities:

Service a maximum caseload of 20 active participants, all of which will be adult women returning from jail/incarceration in Fulton County.

Provide individual case management services with all participants on a weekly or biweekly basis depending on their progress.

Participate in the development of individual reentry plans for all participants that identify barriers to successful reentry.

Assist participants as needed with accomplishing the outlined goals and objectives of their individualized reentry plan by

setting schedules and routines

coordinating services being provided

arranging resources including benefit establishment, resource linkage, and/or, transportation and escort.

Maintain accurate and up to date participants’ records by documenting all participant contact and progress, including time and date, type of contact, outcome, plan of action to include deadlines for completion.

Provide and/or advocate for needed services to holistically address participants’ needs

Maintain relationships and partnerships within the community of local service providers for social service referral needs.

Contribute to all reentry team efforts by communicating with the supervisor and other linked community partners at regular intervals, and developing and maintaining an effective working relationship with all internal and external stakeholders.

Provide regular updates on all active participants to the Reentry Supervisor.

Other duties as necessary provided by Reentry Supervisor or Executive Director.

Minimum Educational/Certification Requirements:

Certification as a Certified Peer through training with Georgia Mental Health Consumer Network, Georgia Council on Substance Abuse or equivalent.

1-2 years’ experience in providing peer support to individuals experiencing unmet mental health needs, substance use disorders, housing instability, and/or justice-involved barriers (reentry services). OR

2-4 years work experience in assisting with re-entry plans of returning citizens.
