Work Schedule :This is a full-time FTE 1.0, day/evening shifts, between the hours of 7:00 am - 10:00 pm, covering approximately 1 in 4 weekends and holiday rotation. Hours may vary based on department needs. Clinical Pharmacist - Inpatient Hematology / Oncology

to:Provide medication therapy management services as part of the interdisciplinary team to patients on inpatient services to assure that drug therapy is safe, effective, efficient, and cost-effective.Provide exemplary patient care in conjunction with interdisciplinary team members in the inpatient settings.Ensure appropriate utilization of medications to optimize patient outcomes.Perform clinical verification of chemotherapy orders.Provide training and education to Pharmacy Residents and PharmD students through practical and didactic experiences.PGY2 in oncology pharmacy or previous work experience in oncology highly desired.At UW Health, you will have
