Speech Language Pathologist

to provide care in our PACE Program. This is an exciting opportunity to join a multidisciplinary care team and support our growing programs for the elderly. Join an existing integrated care team that is collaborative and extremely dedicated to the patients they serve!Responsibilities:Performs evaluations and diagnoses speech and language disorders.Communicates with patient and/or caregivers regarding evaluation results, treatment plan, and goals.Engages patients in the treatment sessions.Provides individual (1:1) treatment to assigned patients as appropriate in accordance with established standards of care and practice.Designs an appropriate and effective treatment plan and works within scope of practice at all times.Monitors progress of treatment goals through periodic observations and assessments. Conducts re-evaluations when indicated.Consults with Physician, allied health professionals, and other team members.Participates in case conferencing meeting and in program planning meetings.Communicates regularly with care team.Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with the department standards; in addition, documentation complies with the patient's insurance requirements.Requirements:Master's degree in Speech Language Pathology required.C.C.C. (ASHA) in Speech Pathology preferred, CPR Certification.Current California State licensure as Speech Language Pathologist required; license must be maintained for duration of employment.Bilingual English/Spanish preferred.Must meet organized driving requirements.Rewards:Job type: Part Time, Per-Diem.In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits or potential bonuses.$40.87 - $52.35If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors, we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
