WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

GENERAL SUMMARY

The Director of Mobile Urgent Care Operations plays a critical role in the health and wellbeing of our patients, as well as our overall goal to reduce unnecessary medical utilization.

The Director of Mobile Urgent Care Operations is specifically responsible for operationalizing our mobile urgent care program by driving appropriate usage and adoption of QuickMedic, our jointly owned mobile paramedic service, to respond to urgent medical patient needs. This leader will work directly with the QuickMedic executive team as well as internal senior leadership including our Regional Market Presidents and Regional Chief Medical Officers to ensure satisfactory execution of our standard operating model.

The successful candidate will help WellBe decrease the cost of care by delivering better, more complete care in the home, and avoiding unnecessary ED/Admits through increased dispatching of our mobile urgent care team for acute issues.

Creates and maintains needed reports for ongoing and continuous performance measurement and improvement

Designs and implements office policies by establishing standards and procedures, measuring results against standards, and making necessary adjustments

Collaborates across organization to plan for, budget, and implement expansions and optimizations

Coordinates internal resources and third parties/vendors for the flawless execution of projects

Supports new market launches ensuring accelerated achievement of QuickMedic go live

Highly collaborative and effective working in a matrixed environment

Excellent communication and organizational skills

Other tasks needed to accomplish team’s objectives/goals.

QUALIFICATIONS

Educational/Experience Requirements:

BA or BS Degree Required. (Master’s degree preferred.)

10+ years of Management experience

10+ years of operational experience. (Healthcare Required)

5+ years of emergency dispatch and/or paramedic experience (Preferred)

Mastery of Excel and PPT are a “must have” for this position.

Travel requirements:

Travel may be required up to 50% locally or nationally

Work Conditions:

Ability to lift up to 20lbs. Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs. Ability to stand for extended periods, Ability to drive to patient locations (ie. home, hospital, SNF, etc), Fine motor skills, and Visual acuity.
