Position SummaryIn accordance with the mission and philosophy of Washington Hospital Healthcare System, the Program Coordinator is a professional caregiver who assists in managing the care for patients and families in the neuroscience program. The program coordinator is expected to work cohesively with neuroscience leadership in the planning, organization, communication, and evaluation of all activities of the program. These responsibilities include developing and maintaining strong relationships with physicians and hospital staff to deliver stroke and spine care to seriously ill and dying patients. This position is accountable for those functions that enhance the delivery of stroke and spine care through activities such as education, standards and program development, quality improvement activities, consultation, and care management. This position is responsible for promoting high quality, cost-effective care and clinical excellence for stroke and spine patients.

In addition to performing the essential functions listed below, may also be assigned other duties as required.

Qualifications

BSN with a valid license to practice nursing in the State of California required

Master’s Degree in Nursing preferred

At least 2 years’ experience in the management and administration of neuroscience related programs is required

Required Certifications: BLS, ACLS, NIHSS and a Nursing Certification in Stroke or Neuroscience

Effective verbal and written communications skills are required

Excel, Microsoft Word, and data management proficiency are required

Requires an effective communicator from a professional and mentoring perspective

Essential Job ResponsibilitiesAchieving Results

Assists in coordinating the provision of stroke and spine care for the organization.

Collaborates to ensure that stroke and spine patients receive a high degree of excellence in care, to maintain TJC Primary Stroke and Spine Program Certification, and assist the neuroscience manager to grow the program.

Maintains an ongoing line of communication among the physician, patient, family/caregiver, and facility staff.

Promotes an interdisciplinary approach to stroke and spine care services within the hospital setting, encompassing the physical, psychosocial, and spiritual elements of care.

Supports all hospital staff as a clinical resource to help solve patient/family issues as they relate to clinical decision making, stroke and spine management as well as ethical issues.

Assists supervisor, hospital staff, hospice staff, etc., to identify and develop strategies to address patient/family needs in complex cases.

Make daily rounds to assure optimal standard stroke and spine patient care is maintained. Collaborate with care providers to assure all of the expectations of a certified primary stroke and spine program are achieved.

Collects and analyzes performance improvement and outcomes data in collaboration with neuroscience manager and the interdisciplinary team that is based on program goals.

Prevent the acquisition of hospital acquired infection by adhering to hand hygiene practice.

Demonstrates Skill

Documents assessment plan, intervention, and evaluation of outcome on the day of service or as needs change during long term interventions. Prioritizes care based on urgency of need.

Participates in the development and ongoing revision of team care plans.

Provides or makes referral for follow up support as needed and conducts appropriate assessments.

Demonstrates personal/clinical skills appropriate to be seen as a hospital-wide expert and resource.

Stays abreast of trends in management and issues related to stroke and spine care including fiscal and managed care issues. Assists in practice changes as appropriate to promote quality and competitiveness in the community.

Assists with hospital and program policies and procedures are updated and appropriate to meet the needs of a primary stroke program and certified spine program.

Demonstrates competency in appropriate areas of applied knowledge, skills, standards, clinical and ethical decision-making.

Demonstrates proficiency for the prompt and impeccable maintenance of the OUTCOME stroke program data registry and American Spine registry.

Maintains confidentiality of all patients and hospital-related information, communications, and documents.

Integrates/applies new knowledge from education and research into clinical practice.

Demonstrates knowledge of competence in stroke and spine standard clinical measures and metrics and assists neuroscience manager for the successful achievement of bi-annual TJC stroke and spine program surveys.

Planning & Coordinating

Provides patients with life threatening illness and their families continuity of care through the illness trajectory, between home, hospital and long-term facility.

Arranges admissions to or delivery of required services of stroke and spine care program.

Participates and assists in coordinating the various aspects of the stroke and spine program such as monthly Stroke Review Committee, Neurovascular Case Studies, Spine clinical operations committee meeting etc.

Collaborates with an interdisciplinary team to plan post-hospital care, focusing on family dynamics, which may influence hospital course and discharge planning.

Consults with the Care Team and primary physician to assess appropriateness of patient care needs.

Participates in delivering education for those in direct contact with stroke and spine patients. This education will be for clinicians in the ED and hospital in general, clinical nurses, and ancillary staff specifically in the areas of stroke and spine care.

Acts as a resource for agencies and organizations providing stroke care.

Assists in facilitating team and family conferences.

Collaborates with the neuroscience manager to provide pertinent stroke education to EMS providers, the community as well as direct the stroke and spine education that patients receive.

Compiles required reports as requested.

Assists in the design and presentation of educational/orientation programs and materials relating to program care.

Collaborates with other team members to participate/coordinate patient/family education and support group programs.

Identifies staff education needs related to provision of, or referral for, appropriate stroke care within the scope of patient care.

Participates in Quality Improvement activities.

Participates in Quality Improvement/Utilization Review initiatives related to stroke care issues.
