RN - Case Manager

Pride Health is hiring RN Case Manager to support our client's hospital facility in Marysville, WA. This is 3 months of contract.Shift Time: 7:30 am-4:00 pm, Mon-FriLocation: Marysville, WA 98271Guaranteed hours: 40 hoursPay Range: $45/hour - $50/hour*Pay offered is based on experience, expertise, and education.Specific Duties:1. Hospital Transition calls for Medicare Advantage patients2. Care Coordination for complex, high-risk patients3. Motivational interviewing with patients and families4. Palliative Care conversations when appropriate5. Participation in Interdisciplinary Team meetingsMust have qualifications/experience:1. Ambulatory Care2. Outpatient Care Coordination3.
