Social Work Intern (Hospital)Bluestone Child & Adolescent Hospital is seeking dynamic healthcare professionals interested in bringing psychiatric care to a new level for children ages 6-17. Working in a brand new, 12 bed state-of-the-art facility, Bluestone staff will have a unique ability to direct patient care, employ best practices, and join a care network that promotes the best long-term outcomes for patients.

INTERNSHIP SUMMARY : Under the administrative/clinical supervision of the Director of Clinical Services/ Field Instructor, the Social Work intern will be responsible for providing social service functions in a healthcare setting. The intern will complete assessments and case management to patients and families. Field hours are flexible, some evenings and/or weekends may apply. SWT or LSW required.

RESPONSIBILITIES INCLUDE:

Perform psychosocial assessments on all patients to determine social service needs, and plan of care.Evaluate patient data, develops and implement a plan of care for the patient.Communicate with physicians regarding social service needs of the patient.Participate in plan of care for each patient; work cooperatively as a member of the interdisciplinary treatment team; completes progress notes and discharge plans in a timely fashion.Provide individual support and facilitate family meetings.Observe and communicate pertinent information regarding patient mental status and behavior, occurrences, and/or negative response to treatment or intervention to physician and/or appropriate health care team members.Maintain current knowledge of social, medical and psychiatric service available within the community; maintains supply of resource materials to be distributed to patients when needed. Is able to obtain other resources as needed.Communicate with family members and caretakers regarding the needs of the patient and anticipated plans.Collaborate with Utilization Review Coordinator and other members of the healthcare team to provide for psychosocial assistance.Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.QUALIFICATIONS INCLUDE:

Currently enrolled in a Master's Degree for Social Work or Counseling required.Previous experience working with youth and adolescents preferred.
