ResponsibilitiesThe Registered Nurse, RN, Case Manager is responsible for the overall coordination of care for an assigned caseload. The goal is to collaborate with patients, families, and healthcare providers to promote health and manage illness. This role facilitates the use of assessment and care coordination for discharge planning transitions.
Knowledge of case management, discharge planning, care coordination, and pre-certification for post-acute care services is needed. 2 years of acute care case management experience is preferred.
QualificationsEDUCATION AND EXPERIENCE:
Associate's degree in Nursing required.
LICENSES AND CERTIFICATIONS:
Registered Nurse in PA – licensed, upon hire, required.
