Health Services Director LPN/RN – Assisted Living

Primary Responsibilities:

The Health Services Director directly oversees the delivery of care to Assisted Living residents to ensure that the emotional, physical, psychological and safety needs are met through all program services. This includes medication management, staff coordination & training, infection control, and applying the nursing process to the care of seniors.

The successful candidate for this position will have:

A passion for seniors and enhancing their quality of life

A Licensed Practical Nurse (LPN) in the State of Missouri; Compact license or ability to obtain is encouraged. An unencumbered license is required.

Proven clinical leader in an assisted living or LTC environment with at least one year of successful experience as a Wellness Director, Health Care Coordinator (HCC) or Director of Nursing (DON)

Understand the importance of being state survey ready, every day

Lead by example and have the heart of a teacher

Committed to team-oriented outcomes and quality care

Excellent oral, computer, and written communication skills.

Be able to multi-task and reprioritize throughout your day

Be able to communicate with all constituents in a professional manner

Have a current driver’s license

Essential Functions:

Lead the clinical team with a sense of purpose and accountability; utilizes metrics and established goals.

Oversee the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers and pharmacy staff.

Identify ongoing needs and services of residents through assessment and service plans utilizing the nursing process.

Visit prospective/current residents in their homes or healthcare setting as needed to assess for admission/readmission to the community.

Act as clinical liaison with families, residents, healthcare professionals and team members.

Direct the infection control measures for the community.

Utilize an EHR system for documentation, medication management and quality initiatives.

Manage labor costs, supplies, and other line items in clinical budget.

Ensure that community follows all federal and state regulations as it pertains to clinical care; including mandatory incident reporting to the State agency.

Responsible for collecting, analyzing, and reporting occurrence trends, and if necessary, develop/implement improvement plans.

Work collaboratively with others on the community leadership team.

Provide, plan, schedule, and monitor staff training programs and in-services for the Assisted Living staff including statutory or regulatory required training. Attend all required training, in-service, and staff meetings.

Manage and coordinate the work of the clinical staff, including overseeing and/or completing staff schedules and daily assignments.

Provide 24/7 on-call clinical support as needed.
