to provide excellent care to patients and families, with the best staff, in the safest environment.The PCA will be responsible for:Assists with meeting direct patient care needsConsistently fosters a healing environment with respect and kindness demonstrating respect for all individuals with sensitivity to cultural diversityContributes to the plan of care through ongoing communication with the RNAssists with patient data and specimen collectionAssists with equipment preparation and set upSupports a Culture of Safety by providing a safe and comfortable patient/family experience where risks are reduced and/or eliminatedCompletes hourly patient care rounds, anticipating patient needs proactivelyMaintains patient and family privacy and confidentialityResponds to patient and family requests, including answering patient call lights in a timely mannerSeeks registered nurse assistance to meet patient/family needs when necessaryCommunicates in a respectful manner with all members of the interdisciplinary teamMonitors patient and environment for safety concerns and immediately reports changes to RN/licensed nurseMaintains a safe and clean patient care environmentFollows Infection Prevention PracticesDocuments aspects of care on approved forms or electronic medical recordParticipates as a member of the Interdisciplinary team to meet patient/family and unit/department needsProactively provides assistance to othersPerforms delegated responsibilities and demonstrates willingness to adapt to changing unit demandsProvides care appropriate to age group of patients on assigned unit(s) as described in the relevant unit profilesMentoring and teaching is provided for new PCAs to meet performance expectationsOther duties as assignedDirect Patient Care:Assists with activities of daily living (i.e. personal hygiene/grooming, bathing/showering, toileting, dressing, and functionality/mobility)Sets up food trays and assist patients with meals as indicatedPerforms preventative skin careAssists patients with positioning, turning, mobilization, ambulation, passive and active range of motion per plan of careApplies/removes pneumatic sequential compression sleevesAdheres to standard precautionsAdheres to hand hygiene guidelinesAssists with admission/discharge processProvides safe patient transportationOrients patients and families/visitors to surroundingsEnsures safekeeping of patient valuables including packing and storingChecks patients for ID bands, and notifies nurse if replacement is neededPatient Data and Specimen Collection:Utilizes 2 patient identifiers for proper patient identification prior to all specimen collectionsObtains and records vital signs, pulse oximetry and non-invasive B/P and immediately reports changes to RN/licensed nursePerforms blood glucose testingEquipment:Retrieves equipment from other areasCleans equipment between patient useDocumentation on Approved Forms and in Medical Record:Records intake of fluids and foodRecords height and weightRecords output of urine, stool, vomitus, and drain outputRecords vital signs, pulse oximetry and noninvasive BPsRecords blood glucose resultsQualifications:Certification as a nursing assistant required upon hire, unless incumbent is a nursing student who has completed her/his fundamentals of nursing with a clinical component (clinical rotation or simulation lab). For nursing students hired as PCAs, strongly prefer nursing students enrolled in BSN programs or direct-entry Masters' programs.Bi-lingual skills preferred.Prior experience in a clinical setting preferred.Knowledge of medical terminology.May not currently hold a valid nursing license.EEO Statement:Brigham and Women's Hospital is an Equal Opportunity Employer.
