Opportunity InformationSutter East Bay Medical Group (SEBMG) is recruiting for a Board Eligible or Board Certified Ambulatory Palliative Care Physician. Provide Palliative Medicine consultation services and participation in Palliative Supportive Care Services interdisciplinary team.Founded in 2005, Sutter East Bay Medical Group (SEBMG) is a rapidly growing multi-specialty medical group whose physicians and advanced practice clinicians work in more than ten care centers and four hospitals in the San Francisco East Bay Area. SEBMG is dedicated to providing exemplary health care to the communities we serve.Job Summary:Provide Palliative Medicine consultation services in multiple settings: Home, SNF, RCFE, Hospital and Office (Palliative Care, Cancer Center, and embedded primary care clinics).Coordinate with subspecialists and the Palliative Supportive Care interdisciplinary team to establish a plan of care.Facilitate access to appropriate supportive care services. Coordinate care with hospital-based palliative care services, outside agencies and local hospices.Assist in the development of palliative supportive care standards, and care tools as well as Quality Improvement and Lean Methodologies.Location:
